Suggested Disability Documentation Guidelines

Individuals with disabilities may send reports from physicians, psychologists, or other professionals which state:
  • The diagnosis and diagnostic criteria or tests used
  • The functional impact of the disability
  • A description of current treatments, medications, assistive devices, and services
  • A description of the expected progression or stability of the impact of the disability over time
  • Recommendations for accommodations
  • The credentials of the diagnosing professionals

The required Disability History Update form allows information to be updated to reflect a student’s current performance or needs.  This form may be completed by the student or someone who knows the student well. More than one person may complete this form and forward it to our office.
